About Home Study and Post Release Services KyUMH works in collaboration with the Office of Refugee Resettlement (ORR) to support children who are placed with caregivers across the state of Kentucky as well potentially contiguous states.&nbsp;The goals of the Home-Study Post Release Services Program (HS/PRS) are to:

<li>Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor’s care (Home Study).</li>
<li>Provide case management support to ensure the continued safety and stability of children in sponsor’s homes and linking children to resources in the community (Post Release Services).</li>

Required Education and Experience&nbsp; Bilingual in English and Spanish (both oral and written) ANDMaster’s Degree in Social Work (MSW) or an equivalent degree in behavioral sciences or social services field, with three years of progressive employment experience in family preservation, kinship care or general child welfare services. <li>Provide initial and continuous training and professional development for HS/PRS case management team regarding responsibilities within the Caminos Program, including agency, state and federal policy updates and changes and best practice service provision.</li>
<li>Provide ongoing and continuous identification of community-based resources, providers and services available to meet the needs of communities served by the program, ensuring that HS/PRS case management team has easy access to current and up to date resources.</li>
<li>Facilitate regularly scheduled individual and group guidance sessions for all members of the HS/PRS case management team, including part time and contract staff.</li>
<li>Actively participate in agency and community efforts to build culturally competent outreach, education, and interventions for the immigrant and refugee community.</li>
<li>Ensure adherence to KYUMH established performance metrics related to best practice contact requirements for youth and family, including phone and in person face to face visitation requirements.</li>
<li>Provide oversight and support necessary to ensure that all case managers submit detailed and thorough documentation in compliance with BCC and ORR requirements and reporting deadlines, including but not limited to: case notes, federal and agency data collection, programmatic reporting, assessments, care plan documentation, transition/discharge planning and post-discharge follow-up.</li>
<li>Act as the primary point of contact for internal and external stakeholders.</li>
<li>Required to maintain a reduced caseload, providing direct service provision of both Home Studies and Post Release Services.</li>
<li>Demonstrates knowledge of and familiarity with both KYUMH and Office of Refugee Resettlement Policy and Procedures relating to services for Unaccompanied Children. This includes knowledge of legal background and authority governing the practice as related to working with Unaccompanied Children.</li>
<li>Demonstrates knowledge of and ability to identify community social service resources and the processes required to connect youth and families to available social service benefits and programs.</li>

A substance abuse nurse practitioner (NP) is a healthcare professional specializing in the assessment, diagnosis, and treatment of individuals struggling with substance use disorders, providing comprehensive care including medication management, counseling, and coordination with other healthcare providers, all while focusing on developing individualized treatment plans for patients in recovery settings like addiction treatment centers or primary care practices; they must possess a deep understanding of addiction psychology, pharmacology related to substance abuse, and be skilled in motivational interviewing techniques to effectively support patients throughout their recovery journey

Essential Functions of an Ultrasound Technologist

<li>Performing general ultrasound exams, including breast, small parts, vascular, and GYN exams in a timely and complete manner </li>
<li>Correlation of clinical history to the test that is being performed </li>
<li>Prepare the exam room and ultrasound equipment to conduct examinations. </li>
<li>Assist radiologists as needed with ultrasound-guided biopsies and vascular ultrasound procedures. </li>
<li>Recognize and record the appropriate anatomy and pathology using sonographic equipment. </li>
<li>Properly care for sonography equipment and report equipment failure. </li>
<li>Write reports based upon the scan results and completion. </li>
<li>Alert appropriate medical staff of any abnormalities that need immediate attention. </li>
<li>Use the ability to solve problems and make decisions. </li>
<li>Work independently or with minimal supervision and as part of a team. </li>
<li>Manage multiple priorities and projects, and display flexibility in a fast-paced work environment. </li>

<li>Provide direct patient care relative to the department organizational policies and procedures, which may include but are not limited to: ECG, echocardiograms, stress echoes, ECG stress test, monitoring, vascular studies, etc.</li>
<li>Document data in the electronic record base (ex: CPACS, Cerner).</li>

<li>Continually monitor and observe the patient during testing and/or procedure and notifies the physician immediately if needed.</li>

What You Need to Bring to the Table:

<li>High school diploma or GED</li>
<li>Current certification as a Registered Diagnostic Cardiac Sonographer (RDCS) by the American Registry for Diagnostic Medical Sonography (ARDMS) OR as a Registered Cardiac Sonographer (RCS) by the Cardiovascular Credentialing&nbsp;International (CCI) required</li>
<li>Current AHA BLS certification required</li>
