Job Description
The Direct Service Professional (DSP) is directly responsible for creating an environment that reflects the mission of RHD's Fernwood and complies with the standards of the Philadelphia's Office of Homeless Services(OHS). The DSP provides direct service to the participants referred to RHD Fernwood as well as providing assistance and information to individuals from external agencies. Essential is the ability to provide information and assist in basic needs to the participants in a calm, consistent manner. Documentation of service along with minor cleaning and upkeep of the common areas is expected. A commitment to the RHD Values should be demonstrated as job duties are performed.

<li>Provide orientation and intake paperwork with Participants and assist volunteers and volunteer groups within RHD/Fernwood</li>
<li>Provide support to encourage resident's effort to accomplish tasks and be available to meet with residents individually as needed.</li>
<li>Answer phone and document messages, activities, observations and other information in logbook, related forms, shift summaries, and other documentation</li>
<li>Serve as a liaison among the residents, case managers, advocates, service providers, OHS and staff</li>
<li>Participate in shift changes, fire drills, and Shelter-in-Place drills along with any other evacuation or emergency incidents to ensure the safety of our Participants and staff.</li>
<li>Medication Monitoring (monitor residents taking medication, counting narcotic medication and reviewing medication books).</li>
<li>Conduct safety checks and be observant in and outside the facility to ensure safety and appropriate behavior.</li>
<li>Intervene in crisis situation and document all critical incidents (Incident Report must be completed within 24 hours of the incident).</li>
<li>Manage the completion and submission of RHD/Fernwood's daily census to Office of Homeless Services.</li>
<li>Conduct bed check and collect signatures for the OHS Monthly Individual sign-in log.</li>

Requirements

<li>High School diploma or GED is required, associates or bachelor’s degree is preferred</li>
<li>Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.</li>
<li>Prior experience in the human services field, preferred.</li>
<li>Valid driver’s license</li>

SUMMARY:
The Residential Aide assist with client basic needs. Individual will work in conjunction with the social services team to monitor and provide appropriate basic services to individual clients. Proof of Covid-19 Vaccinations required.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
· Complete intakes for new clients and provide orientation to the facility and DHS rules and regulations.
· Serve meals and snacks to clients during meal times
· Perform regular area inspections to ensure that shelter rules are adhered too
· Monitor sitting areas, laundry rooms and cafeteria to ensure all clients are adhering to shelter rules
· Maintain a clean and organized storage area
· Document and track locks for all lockers
· Maintain lock log
· Assist with intakes
· Assist clients with securing property within assigned locker
· Store and remove client property from the client storage/locker area as appropriate
· Relocate items and furniture as necessary
· Assist security team as needed
· Cover operations desk when necessary
· Maintain required documentation in case records in writing and computer database (CARES) and update client’s records in accordance with governmental and agency policy.
· Collect and make copies of client identification and other documents necessary during initial intake.
· Assist clients in daily living skills and assist with cleaning area, laundry, hygiene and etc.
· Launder and fold all agency towels, wash cloths and shelter linens after clients have left the facility.
· Follow laundry schedule to ensure that all used linens are washed and stored as appropriate.
· Attend and participate in all required agency meetings: staff meetings, house meetings, in-service training and weekly/bi-weekly case review
· Perform regular inspections of all client areas
· Respond to incidents as appropriate
· Answer phones when necessary
· Assist with all house operations
· Assist with maintenance duties as needed
· Review logbooks at start of each shift to ensure that all new client intakes are completed within 24 hours as required.

SCOPE OF RESPONSIBILITY:
The Direct Support Staff is responsible for all aspects of resident care as well as for routine maintenance of the residence.
 WORKING RELATIONSHIP:
The Direct Support Staff reports directly to the Manager also the Lead Direct Support Staff on duty for all aspects of the job responsibilities. Develops and maintains relationships with the following:

<li>Residents, Manager, Lead Direct Support Staff, co-workers, Agency Nurses, Agency Behaviorist, residents’ family and/or guardians, Healthcare Consultants, DDS employees, and people in the community. </li>

QUALIFICATIONS:

<li>High School diploma or GED is required.</li>
<li>Obtain and maintain medication certification.</li>
<li>Good interpersonal relationship and communication skills.</li>
<li>Maturity, good judgment, and ability to handle confidential information.</li>
<li>Must be proficient, or become proficient, and maintain proficiency in First Aid, CPR, PMT, Abuse/Neglect, DDS Medication Administration and other resident's specific medical and habilitative needs. Attend all required in-services and staff meetings. Review and follow all agency policies and procedures.</li>
<li>Competent in basic computer skills, experienced with electronic health records is preferred.</li>
<li>Valid Connecticut Driver’s License and eligibility for Network, Inc.'s auto insurance.</li>
<li>Must be physically able to assist participants when necessary (i.e. lifting, using approved adaptive equipment, using approved physical restraint techniques).</li>
<li>Must be proficient, or become proficient, and maintain proficiency in the electronic health record system THERAP.</li>

<li>daily resident specific task analysis charts</li>
<li>other daily resident specific records as directed</li>
<li>ongoing inter-staff communication in the T-logs</li>

5. Participates in the normal family routine of meal preparation, as needed and assigned.

10 Maintains resident confidentiality and resident record confidentiality.
11. May be requested to cover coworkers’ vacation, sick, or other planned/unplanned absences from work as well as additional emergency staffing including being frozen in on shift.
