Position Description 
Provides personal care services in accordance to the Individual Support Plan (ISP). The Direct Support Professional is assigned to specific Individuals and performs services for Individuals, as necessary, according to their ISP to maintain their personal comfort, promote independence within the community and achieve an everyday life.
Essential Job Functions/Responsibilities 
1. Performs simple procedures according to Individual Service Plan. 
a. Assists individuals with daily living skills and community skills. 
b. Utilizes Person Centered Practices to assess individual needs. 
c. Completes training with individuals towards achieving ISP goals. 
2. Performs personal care activities, including but not limited to: 
a. Bathing 
b. Shampooing/hair care 
c. Skin care/nail care 
d. Oral hygiene 
e. Shaving 
f. Dressing 
g. Toileting/elimination
h. Catheter care
3. Provides support in areas of independent/home-living such as: 
a. Meal preparation/feeding/fluid intake 
b. Provides assistance in in the individuals laundry. 
c. Light housekeeping in areas directly related to the individual’s living area. 
d. Shopping as related to the individual’s community needs and preferences.
4. Reports any observed or reported changes in the individual’s condition and/or needs to the Executive Director or designee.
5. Documents care provided and completes daily progress notes. Completes the appropriate records to document care provided. Submits documentation at least weekly.
6. Promotes personal safety and a safe environment for individuals by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Executive Director or designee.
7. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.
8. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested. 
9. Maintains confidentiality in all aspects of the job.
10. Attends office required programs to meet compliance requirements. Identifies learning needs to supervisor.
11. Performs other related duties and responsibilities as deemed necessary.

Mental Health Tech / Direct Support Professional

<h3>Job Description</h3>
Job DescriptionFull-time, Part-time and PRN for Memory Care Community Essential Job responsibilities: 1. Monitor the safety and well-being of the residents. 2. Report and document resident concerns or changes of condition to the appropriate medical provider, resident’s family, and the Clinical Director. Document in EMAR as appropriate. 3. Complete routine assessments and document in EMAR. 4. Administer Medications to residents according to Physician orders and following facility policies. Correctly document administration in the EMAR. 5.Check in new medications and ensure correctly documented in the EMAR for correct administration by other staff. 6. Assist resident assistants with activities of daily living including dressing, bathing, toileting, hygiene routines, meal preparation, and mobility when necessary. 7. Assist with prepping meals for all residents, setting tables, bussing tables, and mealtime assistance. 8. Encourage residents to socialize with others and participate in activities as appropriate. 9.. Assist with program activities, encouraging residents by assisting with projects or following instructions provided by the Activities Director or supervisor. 10. Chaperone activities outside the unit when necessary. 11. Report changes or concerns in resident condition or status to the Nurse or Clinical Director. 12. Document change in condition in Service Minder or Notes in EMAR. 13. Assess and report vital signs as assigned. 14. Report changes in functional status or obvious injuries to Nurse or Clinical Director. 15. Maintain professionalism with other staff, administration, and residents in all interactions. 16. Maintain good communication with co-workers, administration, families, and residents. 17. Maintain a clean and safe environment for staff, residents, and families. 18. Provide personal support to the residents when dealing with the stressors of dementia. Use a calm voice to reassure and redirect as necessary. 19. Document daily activities, intake, output, and assistance given in Service Minder. 20. Follow Policy and Procedures for admissions, change of conditions, and other unit functions. 21. Respond to resident emergencies; provide first aid; arrange for further medical evaluation and transport if necessary. Expected Working Behaviors: 1. Maintain professionalism 2. Adhere to facility Policies, Procedures, and Standards of Practice. 3. Maintain confidentiality of verbal and written communication with all facility operations and resident matters. 4. Represent the facility professionally when representing the unit outside of The Meadows. 5. Promote teamwork. 6. Adhere to safety guidelines. Knowledge: 1. Fire and Emergency Procedure 2. Food handling 3. Unit Policies, Procedures, and Standards of Practice. Qualifications: 1. License or Certification 2. Experience with dementia care preferred. 3. Experience with elderly preferred. 4.
