<li>Appropriately consults with the attending or on-call physician concerning alteration of the plan of treatment and documents, in writing, appropriate change of orders where necessary.</li>
<li>Communicates with supervisor concerning problems of the patient(s) on a timely basis and communicates information using current process and technology available to the organization.</li>
<li>Turns in all paperwork to the designated person in the office by 5pm on Monday, and syncs ipad nightly when visits are done. Makes a verbal report of new admissions and ER visits to the primary nurse Sunday night or Monday morning between 8:00-9:00 a.m.</li>

<li>Work with the medical team to develop specific clinical and behavioral health pathways for targeted patient groups (asthma, diabetes, heart disease, hypertension, obesity etc.).</li>
<li>Provide ongoing consultation services for a sub-set of patients who require ongoing monitoring and follow up (e.g., Collaborative Care Model).</li>
<li>Document all patient care in the proper systems and in a timely manner in compliance with organizational policies and regulatory agencies.</li>
<li>Familiarize self with COPA organizational structure, communications systems, billing process, and completes billing and coding documentation within time assigned. </li>
