Location: Hills & Dales Community Center 3505 Stoneman Rd Dubuque, IA 52002
Schedule: Monday-Friday, 8 hour days between 7AM-6PM. MUST be present during the hours of 10AM-2:30PM during the day hab program setting.

About the Role
The Intermittent & Day Habilitation Program Coordinator oversees the operations of both the Intermittent and Day Habilitation Programs. This position focuses on program development, quality services, staff supervision, and compliance with all regulatory standards. You'll ensure individuals in our programs experience growth, independence, and meaningful community participation.
What You'll Do

Oversee daily operations of both programs, including program development, scheduling, and evaluation.

Support the Community Services Director with strategic planning, curriculum development, and program growth.

Supervise and train program staff to ensure high-quality, person-centered supports.

Develop and implement individualized goals and activities that promote community integration and skill-building.

Maintain compliance with all HCBS, CARF, Medicaid, and agency standards.

Review documentation, monitor quality, and ensure accurate reporting.

Coordinate transportation and ensure staff and client safety.

Participate in team meetings, contribute to client plans, and provide direct support as needed.

Experience supervising or managing staff in a human services or similar setting.

Knowledge of HCBS, Medicaid, or CARF standards preferred.

Ability to manage multiple priorities and work collaboratively with a team.

Proficient computer and documentation skills.

Bachelor's degree in Human Services, Recreation Therapy, or related field preferred (experience may substitute for education).

Valid driver's license and ability to meet agency driving requirements.

Must be able to complete and maintain certifications in Medication Management, CPR, First Aid, and QBS (training provided).
Physical & Work Requirements

Ability to lift up to 35 lbs and actively support individuals as needed.

Comfortable working in a dynamic environment with varying activity levels.

Flexible schedule and willingness to be part of an on-call rotation.
