Direct Primary Care/Family Medicine practice in Boulder looking for an exceptional, experienced Physician Assistant or Nurse Practitioner. We are a full spectrum family medicine practice that focuses on personalized, high quality care. We are looking for a mid level provider who thrives on patient-focused visits. This includes comprehensive histories, detailed physical exams and thorough patient plans and follow up care. We strive to provide the best quality care to our patients by maintaining smaller patient panels and longer visits ranging from 30 to 120 minutes.
Hours: 1 day/week with future possibility of full time employment.
ESSENTIAL RESPONSIBILITIES:
Data Collection
- Perform complete, detailed and accurate histories, review patient records and order laboratory, radiological and diagnostic studies appropriate for patient's history and physical condition .
- Perform complete physical examinations and record pertinent data.
- Perform appropriate in office studies including labs and diagnostic studies.
Assessment
- Make medical diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, other resources of the community, or other physicians.
Treatment Planning
- Develops and implements treatment plans including prescribing medications, sending orders, referrals, additional testings and appropriate follow up in accordance with the practice’s preferences.
Procedural
- Performs procedures within the limits of their expertise and in accordance with the State Regulations and Organizational Policies.
Coordination of Care and Education
- Educate and coordinate care with patients and families.
Other Tasks
- Maintain HIPAA compliance and use Universal Precautions.
- Conducts telephonic and virtual encounters as appropriate.
- Manage lab, imaging study and other diagnostic procedure results in timely manner.
- Contribute to a collaborative office environment.
- Know their limitations and when to seek advice or assistance from other team members.
PROFESSIONAL EXPECTATIONS
- Maintain all appropriate licensure and certifications.
- Stay up to date on clinical competence through appropriate continuing education and other activities.
BASIC QUALIFICATIONS
Experience
3+ years or experience
Education/ Certifications
- Family Nurse Practitioners
- Master's Degree in Nursing
- Graduation from an accredited graduate nursing program approved by the Colorado State Board of Nursing.
- APRN registration with the state of Colorado
- Licensed as registered nurse in the state of Colorado
- National Provider Identification (NPI) and Taxonomy codes required at time of hire
- Active DEA (full prescriptive authority)
- Active BLS or ACLS
- Physician Assistants:
- Bachelor's Degree
- Graduation from a program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA)
- Licensed as a physician assistant in the state of Colorado
- National Commission for the Certification of Physician Assistants (NCCPA): certification required
- National Provider Identification (NPI) and Taxonomy codes required at time of hire
- Active DEA (full prescriptive authority)
- Active BLS or ACLS
