Job Title: Housing Coordinator

Job Summary:
The Housing Coordinator is responsible for facilitating the application process and transitioning clients into affordable housing. This role also provides ongoing support services to ensure housing stability, including advocating for clients and managing housing retention efforts.

Duties and Responsibilities:

Intake, Assessment, and Documentation:

  • Respond to referrals and new client inquiries in a timely manner.
  • Complete client intake forms and assessments, determining needs and preferences.
  • Maintain up-to-date client case files, ensuring all documentation is organized and accurate.
  • Keep detailed case notes in CHHC’s database, ensuring client data, including NPI numbers, is accurate and current.
  • Communicate regularly with clients, case managers, service coordinators, service providers, and support personnel to ensure housing application requirements are met and housing stability is maintained.

Information and Assistance to Clients:

  • Identify and present housing options that meet client location, size, and affordability requirements.
  • Assist clients in understanding and signing lease agreements.
  • Help clients, along with their support staff and family members, complete applications and provide necessary documents to be placed on affordable housing waiting lists and for Housing Choice Vouchers.
  • Assist clients in requesting reasonable accommodations based on disability or medical needs.
  • Help clients qualify for housing by making referrals for credit counseling or legal assistance, assembling letters of support, and applying for eligible financial assistance.
  • Support clients in budgeting for move-in expenses, including the security deposit and first month's rent, by applying for financial assistance programs.
  • Be available to answer questions from clients, family members, and authorized service providers.
  • Facilitate the housing application process, maintain waitlists, show properties to clients, and assist selected clients in submitting the required documents.
  • Ensure clients maintain housing by assisting with lease renewals, lease violations, or re-certifications.

Community Engagement and Landlord/Property Manager Relationships:

  • Represent CHHC professionally in community meetings and build relationships with landlords and property managers to expand housing options for clients, particularly those with developmental disabilities.
  • Maintain an updated list of affordable housing/rental properties accepting waitlist applications.
  • Regularly communicate with landlords and housing developers to keep the property database current and share application forms and requirements.
  • Identify housing advocacy opportunities and collaborate with the Housing Development Advocate and Executive Director to pursue them.
  • Advise clients and landlords about the amount of funding approved for assistance and follow up to ensure immediate housing issues are resolved.
  • Perform other duties as needed.

Post-Move Support:

  • Offer follow-up services to clients after they move to ensure housing stability.
  • Advocate on behalf of clients with community resources to prevent eviction and ensure safety.
  • Manage client behaviors that could jeopardize housing stability.

Transportation and Coordination:

  • Provide in-service transportation when necessary.
  • Collaborate with public benefit programs, employment opportunities, and landlords to promote housing retention.

Recordkeeping and Compliance:

  • Maintain accurate and detailed progress notes as required.
  • Participate in weekly team and individual meetings as required.
  • Ensure compliance with Minnesota Healthcare programs and processes, adhering to HIPAA and Data Privacy Practices.

Requirements and Abilities:

  • Completion of Housing and Community Development (HAC DHS) training within the first week of employment.
  • Completion of mandated reporter training for vulnerable adults within the first week of employment.
  • Successful passing of a Department of Human Services (DHS) background check.
  • Proficiency in person-centered thinking and practices.
  • Knowledge of the housing search process and various subsidized housing options, including LIHTC, Section 8, and other assistance programs.
  • Exceptional verbal and written communication skills.
  • A reliable vehicle, valid driver’s license, and current insurance with a great driving record.
