JOB DESCRIPTION
JOB TITLE: Registered Nurse/PT
DEPARTMENT: Supportive Housing
FLSA: Non-Exempt
LOCATION: Bronx, NY
REPORTS TO: Service Coordinator
SALARY: $60,000.00 to $ 65,000.00/Annual
SUMMARY: A key component of this job is to support and promote the residents’ independence by establishing care goals; teaching the residents to understand their condition, medications, and self-care skills.
DUTIES & RESPONSIBILITIES:
- Performs nursing assessment of residents upon program entry, and annually thereafter. Identifying any risk factors, and providing care for any presenting /outstanding medical and/or psychiatric issues.
- Assess the health needs of all recipients and make appropriate medical and mental health referrals as needed.
- Assists with medical and/or psychiatric emergencies immediately upon notification and following up to ensure recovery.
- Participates in the development of residents Individual Support Plans (ISPs).
- Maintains accurate, detailed reports and records symptoms or changes in residents’ conditions.
- Documents encounters with the residents on a progress note and filing notes in a timely manner.
- Interprets and evaluates diagnostic tests to identify and assess residents’ condition and shares information with the team.
- Consults and coordinates with residents’ Psychiatrist/ behavioral health and healthcare providers to assess, plan, implement or evaluate residents’ care plans.
- Monitors all aspects of residents’ care, including diet and physical activity.
- Assist clients with pre-packing medications and education around side effects as needed.
- Facilitates groups for the residents on topics such as health education, medication management, symptoms of mental health, disease prevention
- Assist the residents and service staff to develop health improvement programs to promote good health practices.
- Maintains nursing supplies inventory by checking stock to determine inventory level; anticipating needed supplies; placing and expediting orders for supplies.
- Some evening and weekend work may be required.
- Other duties as required or assigned by supervisor.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
Successful applicant must have strong written and verbal communication skills; knowledge of medical and psychological terminology; provide good customer service, both in person and by telephone; must possess good organizational skills, have a high attention to detail, have the ability to handle multiple tasks, prioritize work assignments, and maintain confidentiality. Bi-lingual Spanish a plus. Must have at least 2 years of experience working with substance abuse, mental health and homeless and population. A Bachelor’s of science in nursing and current New York State RN License and Registration required.
