Job Description
We are seeking a Home Health Speech Language Pathologist to conduct home visits, assess, and treat speech, language, social communication, cognitive, and swallowing disorders.
- Perform initial assessments, develop and revise individualized care plans, and coordinate with healthcare team members.
- Monitor patient progress, document visits, and report to physicians, ensuring timely re-evaluations every 30 days.
- Educate patients and caregivers to promote independence and understanding of impairments.
- Qualifications include a Master's degree, active SLP license (CCC), and at least one year of experience, with home health documentation skills.
