Job Summary:

Contribute to developing, implementing, monitoring, and updating the individualized rehabilitation plan. Perform therapeutic rehabilitative skill development, and offer services appropriate to meet the consumers' needs.

DUTIES

• Allocate the majority of hours worked toward face-to-face contact with assigned consumers.
• Develop comprehensive service plans that identify specific goals consistent with the consumer's wellness and recovery and the agency's mission.
• Work cooperatively with other social service agencies, health providers, and mental health providers to render quality service through a collaborative treatment approach.
• Transport Consumers.
• Document service contacts, service plans, and service coordination activities on a web-based documentation system.

QUALIFICATIONS

• A Bachelor's Degree in Social Work, Psychology, or other human services-related degree is required
• Valid non-provisional NJ Drivers License with not more than 5 points required
• A minimum of one year of experience working with individuals with a mental illness in a residential setting is required
