GENERAL DESCRIPTION:
The function of the Qualified Professional (QP) is to provide support and training to the Associate Professional, Para-Professionals, Certified Peer Support Specialist, Vocational Specialist, Habilitation Specialist, Home Manager, and Direct Care Staff. The QP also serves as a liaison between family members and the facilities as well as other outside agencies to ensure continuity of care for all consumers that are being served by Greater Image Healthcare.
SPECIFIC DUTIES AND RESPONSIBILITIES: (Not limited to those listed)
- Coordination and oversight of initial and ongoing assessment activities.
- Conduct monitoring observations to ensure proper implementation of each consumer’s Person-Centered Plan (PCP, and ongoing revision of PCP.
- Provide initial assessment and the development of the PCP
- Conduct monitoring observations to ensure proper implementation of each client’s Individual Program Plan (IPP).
- Review data; monitor and review quarterly documentation completed by Habilitation Specialist
- Provide supervision to AP’s and Paraprofessionals on a monthly basis and/or as needed.
- Therapeutic mentoring, symptom monitoring, monitoring medication and self-management of symptoms.
- Provide in-service training as needed.
- Conduct school observations as needed.
- Provide one-to-one interventions with the consumer to develop interpersonal and community and relational skills.
QUALIFICATIONS:
· An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a Registered Nurse who is licensed to practice in the state of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SA with the population served;
OR
· A graduate of a college or university with a Master’s Degree in a Human Service field with at least one year of full-time, post-graduate degree accumulated MH/DD.SA experience with the population served, or a substance abuse professional with less than one year of full-time, post-graduate accumulated supervised experience in alcoholism and drug abuse counseling;
OR
· A graduate of a college or university with a Bachelor’s Degree in a Human Service field with at least two years of full-time, post-bachelor’s degree accumulated MH/DD.SA experience with the population served, or a substance abuse professional with less than two years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling;
OR
A graduate of a college or university with a Bachelor’s Degree in a field other than Human Services with at least four years of full-time, post-bachelor’s degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional with less than four years of full-time, post-bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling
