ABOUT THE TEAM ????

The Supported Living (SL) team provides support for a variety of men and women, within a time 24 hours a day time frame, seven days a week if needed. Supports range from health, safety, and community participation for those with more significant needs. The team also provide services for persons in his/her home to include health, safety, and community participation as well as assisting in coordinating the activities of the home by eliciting the preferences and needs of and providing support to persons.

The PRN DSP is responsible for the emergency coverage of the Supported Living Direct Support Professional shifts. The PRN DSP will work with people who are supported in person. The PRN DSP is a part-time employee who reports directly to the Support Coordinator. Required to work 2 shifts minimum per month.

IN THIS ROLE, YOU'LL: ????

  • Provide emergency direct support coverage for DSPs
  • Aid the person you will support with personal care, medical, behavioral, or safety needs.
  • Provide personal care to include eating, using the bathroom, bathing/showering, transferring, shaving and personal hygiene issues.
  • Conduct quality service checks to include conditions of home, personal care, adequate household, and personal supplies
  • Proactively implement systems to ensure quality services for persons in designated areas such as health, safety, and community participation.
  • Track, book, and accompany persons to medical appointments, ensuring that required medical documentation is completed.
  • Research for community resources and other resources (i.e.: education, spiritual, leisure, transportation and medical).
  • Facilitate successful participation in events, including staff training, community education, technology, and resource development.
  • Collaborate with Team Coordinators and staff to ensure people's transportation, accommodations, etc. for the people you support.
  • Perform other duties as assigned

ABOUT YOU ????

  • High School Diploma (AA preferred)
  • Experience supporting people with developmental disabilities
  • Must have knowledge and familiarity of the surrounding counties
  • Must have a valid driver’s license and reliable transportation to work
  • Strong analytical skills, good mentoring and coaching abilities and professional approach to personal development.
  • Must be willing to learn and use technology, such as cell phones, iPhone apps, photo, videos, laptops, adaptive technology, and case management database
  • Must possess an excellent performance ethic and be a strong team player
  • Must complete required training (e.g. CPR/first aid, safety protocols, CMT, MANDT)
