Employment Details: Regular, Full Time

Salary Range: $44,500 - $62,900

Education/Experience: Bachelor’s Degree with at least 2 years post graduate experience in a social work-related field or a Master’s Degree or higher in a social work or related field.

Position Description

HOPE is seeking a full time Treatment Foster Care Specialist who will

contribute to HOPE’s mission by providing safe and nurturing foster homes and case

management services to children and youth in out of home care. Through careful

assessment and identification of children’s emotional and behavioral needs, Treatment Foster

Care Specialist is able to deliver services and supports consistent with our vision of giving

children and families HOPE in realizing their potential and set a path for their success.

Duties and Responsibilities

· Ensure treatment foster parents are meeting their responsibilities according to the H.O.P.E Foster parent Manual and the Wisconsin Administrative Code DCF 56 Care for Children.

· Provide in home support, consultation, counseling and case management services for

9 -12 children/youth placed in Treatment foster care. Also, provide services to their families and treatment foster parents.

· Ensure all treatment foster children and treatment foster parents needs and concerns are addressed and met in a timely manner.

· Provide crisis intervention, including responding to needs and after hour services based on team coverage schedule.

· Assessment and completion of Initial (within 30 days of placement) and quarterly treatment plans.

· Completion of client progress notes for every contact with the treatment foster child, treatment foster parent and team member.

· Provide discharge planning and aftercare services for children in care.

· Collaborate with child’s treatment providers and other professionals.

· Attend and participate in relevant court and/or school appointments, medication reviews, team meetings.

· Assist in facilitating and locating respite providers for children in care.

· Conduct foster parent evaluation on an annual basis.

· Assist in the licensing process, including SAFE re-assessments for currently licensed foster parents.

· Review monthly with foster parents their training plan to assure timely completion of ongoing training.

· Support the H.O.P.E. mission and vision and represent the agency professionally and positively at all times.

· All other duties as assigned by Executive Director or Program Director.

Knowledge, Skills and Qualifications

· Bachelor’s degree in Social Work or related field with 2 years post graduate experience.

· Master’s Degree in Social Work or related field.

· Licensing or certification required per ch. 457, Stats., ch MPSW 3, 6, 11, 12, or 17

· Experience with SED population preferred.

· Complete orientation and training requirements provided and/or offered by H.O.P.E. (DCF 56, DCF 54, Prudent Parenting, SAFE Assessment, Trauma Informed Care and other relevant trainings).

· Possess cultural competence by presenting respectfully and effectively to people of all cultures, languages, classes, races, ethnic backgrounds, religions and other diversity factors in a manner that recognizes, affirms, and values the worth of each individual.

· Proficient in computer skill with the ability to use the Internet, Microsoft office products, Excel, etc.

· Demonstrate good verbal and written communication.

· Ability to work cooperatively in a team approach.

· Ability to communicate effectively with children, families, staff and other professionals.

· Ability to work a flexible schedule, some evenings and weekends required on a rotating schedule.

· Valid driver’s license.
