Work Shift: Day (United States of America)

Salary Range: $47,921.64 - $71,882.46 (Hourly: $27.00/hr. - $37.32/hr.)

Your Role:

As an LPN Specialty Coordinator, you will be the heartbeat of our outpatient specialty services. Your responsibilities will include:

  • Coordinating scheduling and care needs for patients requiring specialty physician services.
  • Anticipating patient needs and ensuring the availability of necessary equipment and supplies.
  • Providing clinical support during office visits, including patient rooming and chart updates.
  • Traveling to outside facilities for provider support as directed.

Essential Duties and Responsibilities:

  • Obtain and accurately record patients' vital signs.
  • Document components of patients' history and participate in holistic assessments.
  • Collaborate with the healthcare team to integrate assessment data into care plans.
  • Demonstrate a caring and empathetic approach to individualized patient care.
  • Assist in the coordination and implementation of personalized care plans.
  • Communicate openly and appropriately with patients, visitors, and staff.
  • Participate in in-office procedures under the supervision of licensed healthcare providers.
  • Prepare patient charts with pertinent clinical information.
  • Maintain inventory of supplies and ensure examination rooms are stocked.
  • Adhere to regulatory compliance standards.

Qualifications:

  • High School Diploma/G.E.D. required.
  • Must hold and maintain a NYS Registration as a Licensed Practical Nurse.
  • Certification in Basic Life Support (BLS) is required.
  • Previous experience in a patient care setting is preferred.
  • Excellent verbal and written communication skills.
  • Ability to effectively present information and respond to questions from patients and staff.
