MLTC Intake Nurse – RN
Location: Brooklyn, NY (Hybrid)
Language Skills: Chinese or Spanish a strong plus
Salary: $95,000 – $105,000 annually
Employment Type: Full-Time

A mission-driven Managed Long Term Care (MLTC) program serving the diverse communities of Brooklyn is seeking a Registered Nurse to join its Intake team. This is a hybrid role requiring field-based assessments and in-office collaboration.

The MLTC Intake RN plays a vital role in evaluating potential new members for program eligibility, educating them and their families on available services, and supporting a smooth transition into long-term care. Bilingual language skills in Chinese or Spanish are highly preferred due to the populations served.

Key Responsibilities:

  • Conduct in-person assessments using the UAS-NY tool to determine program eligibility and safety for potential members.

  • Educate prospective members and families about the MLTC program, services, and enrollment process.

  • Collaborate with the Intake/Outreach and Clinical teams to coordinate timely, appropriate onboarding of new members.

  • Communicate professionally with referral sources, physicians, caregivers, and community partners.

  • Participate in outreach activities to support community engagement and referrals.

Qualifications:

  • Active New York State RN license required.

  • Bachelor’s degree in Nursing required.

  • Minimum 1 year of acute care experience.

  • Proficiency in UAS-NY assessments required.

  • Experience in home care and care management preferred.

  • Strong communication and interpersonal skills; fluency in Spanish or Chinese is highly desirable.

  • Proficiency in Microsoft Office applications.
