Job DescriptionDay Habilitation Program counselor

  1. Qualifications
  1. High School Diploma or GED required
  2. Work experience in driving routes.
  3. Experience with driving children or adults with developmental disabilities preferred
  4. Sensitivity to the needs of all consumers
  5. Certification by the Department of Motor Vehicle, State of New York as to an acceptable driving record and valid driving credentials
  6. Strong command of verbal and written communication (English)
  7. Approval, without restrictions, by the facility’s insurance
  1. Accountability
  1. Directly accountable to the Transportation Coordinator and Day Habilitation Program Coordinator
  1. Counselor
  1. Responsible for implantation and collection of data of daily and weekly programs as assigned by Specialist for each consumer in assigned group
  1. Assists in any clerical work (i.e. daily attendance, requisitions, trip lists, graphing data)

C. Implements weekly and daily programs to assist each consumer in attaining their individual goals

  1. Documents consumers progress in increase and decrease programs on a daily basis (collects and graphs data)
  2. Responsible for participating in behavior analysis training program
  1. Specific Responsibilities
  1. Directly responsible for transporting consumers as assigned by Transportation Coordinator
  2. Responsible for transporting consumers from home to day program, and home again in safe, efficient manner
  3. Responsible for driving consumers on all planned trips to community outings.
  4. Promotes the safety and well being of adults and staff while transporting
  5. Oversees all maintenance responsibilities for Day Habilitation vehicle(s) as assigned:

-makes sure vehicle registration, inspection and insurance are current and necessary sticker, copies are on/in vehicle

-First Aid and Roadside Assistance kits in vehicle(s) and properly stocked

-remove any garbage from vehicle following each trip, clean any soiling (vomit, spills etc.)

-once a week, or more often if necessary, wash outside vehicle, wash all seats, interior walls, dashboard, railing and interior windows of vehicle(s)

  1. Inspect vehicle before first scheduled departure and complete a Daily Vehicle Inspection Sheet and hand in to the Transportation Coordinator
  2. Ensures Day Habilitation vehicles are in safe, operable, clean condition by ensuring proper maintenance.
  3. Complete and submit daily, weekly and monthly reports in a timely manner.
  1. Assist with training new drivers.
  2. Assists in training new driving staff on Agency Vehicle Policy & Procedures
  3. Implements on-going preventative maintenance programs on agency vehicles
  4. Attends on-going vehicle safety education/training
  5. Coordinate the vehicle(s) repairs/maintenance visits with Transportation Coordinator and Repair Shop.

5. General Responsibilities

  1. Participates in training sessions and in-service meetings
  2. Insures that program areas are kept in order
  3. Assumes additional responsibility when assigned to maintain the quality of care for all individuals
  4. Follows established Hawthorne Foundation Inc. policies and procedures
  5. Participates in all recreational programs
  6. Assures the health, safety and welfare of all individuals
  7. Assumes additional responsibilities as assigned
  1. Physical Responsibilities
  1. Sitting, standing, bending, lifting, and moving intermittently during work hours
  2. Must be able to cope with the mental and emotional stress of the position
  3. Must possess sight and hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met
  4. Must function independently, have personal integrity, be flexible and able to work effectively with other personnel
  5. Must be in good general health and demonstrate emotional stability
