Job DescriptionDay Habilitation Program counselor
- Qualifications
- High School Diploma or GED required
- Work experience in driving routes.
- Experience with driving children or adults with developmental disabilities preferred
- Sensitivity to the needs of all consumers
- Certification by the Department of Motor Vehicle, State of New York as to an acceptable driving record and valid driving credentials
- Strong command of verbal and written communication (English)
- Approval, without restrictions, by the facility’s insurance
- Accountability
- Directly accountable to the Transportation Coordinator and Day Habilitation Program Coordinator
- Counselor
- Responsible for implantation and collection of data of daily and weekly programs as assigned by Specialist for each consumer in assigned group
- Assists in any clerical work (i.e. daily attendance, requisitions, trip lists, graphing data)
C. Implements weekly and daily programs to assist each consumer in attaining their individual goals
- Documents consumers progress in increase and decrease programs on a daily basis (collects and graphs data)
- Responsible for participating in behavior analysis training program
- Specific Responsibilities
- Directly responsible for transporting consumers as assigned by Transportation Coordinator
- Responsible for transporting consumers from home to day program, and home again in safe, efficient manner
- Responsible for driving consumers on all planned trips to community outings.
- Promotes the safety and well being of adults and staff while transporting
- Oversees all maintenance responsibilities for Day Habilitation vehicle(s) as assigned:
-makes sure vehicle registration, inspection and insurance are current and necessary sticker, copies are on/in vehicle
-First Aid and Roadside Assistance kits in vehicle(s) and properly stocked
-remove any garbage from vehicle following each trip, clean any soiling (vomit, spills etc.)
-once a week, or more often if necessary, wash outside vehicle, wash all seats, interior walls, dashboard, railing and interior windows of vehicle(s)
- Inspect vehicle before first scheduled departure and complete a Daily Vehicle Inspection Sheet and hand in to the Transportation Coordinator
- Ensures Day Habilitation vehicles are in safe, operable, clean condition by ensuring proper maintenance.
- Complete and submit daily, weekly and monthly reports in a timely manner.
- Assist with training new drivers.
- Assists in training new driving staff on Agency Vehicle Policy & Procedures
- Implements on-going preventative maintenance programs on agency vehicles
- Attends on-going vehicle safety education/training
- Coordinate the vehicle(s) repairs/maintenance visits with Transportation Coordinator and Repair Shop.
5. General Responsibilities
- Participates in training sessions and in-service meetings
- Insures that program areas are kept in order
- Assumes additional responsibility when assigned to maintain the quality of care for all individuals
- Follows established Hawthorne Foundation Inc. policies and procedures
- Participates in all recreational programs
- Assures the health, safety and welfare of all individuals
- Assumes additional responsibilities as assigned
- Physical Responsibilities
- Sitting, standing, bending, lifting, and moving intermittently during work hours
- Must be able to cope with the mental and emotional stress of the position
- Must possess sight and hearing senses, or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met
- Must function independently, have personal integrity, be flexible and able to work effectively with other personnel
- Must be in good general health and demonstrate emotional stability
